Mechanism of Action
Echothiophate iodide acts as an irreversible inhibitor of acetylcholinesterase, binding covalently to the enzyme's active site. By inactivating cholinesterase, it prevents the hydrolysis of endogenously liberated acetylcholine in parasympathetically innervated ocular structures.
Application
Echothiophate iodide is indicated for the treatment of open-angle glaucoma and accommodative esotropia. In glaucoma management, it provides effective round-the-clock intraocular pressure control, though it is now reserved for refractory cases due to its side effect profile. For accommodative esotropia in pediatric patients, it is used diagnostically and therapeutically to correct ocular deviation.
