Desflurane-based anesthesia provided neurologic protection in a piglet model of deep hypothermic circulatory arrest. Arterial desflurane concentrations reached nearly 9 percent within 10 minutes during cardiopulmonary bypass cooling and decreased rapidly to approximately 2 percent within 10 minutes after drug withdrawal during rewarming. The desflurane group demonstrated significantly better neurologic performance scores on postoperative day 2 compared to the fentanyl group. Histologic examination revealed less tissue damage and fewer dead neurons in the neocortex of desflurane-treated animals. These findings demonstrated that desflurane conferred neurologic protection against ischemic injury associated with CPB and DHCA, suggesting potential application in infant cardiac surgery to reduce neurologic complications.
