General Description
Desflurane is a fluorinated inhalation anesthetic of the halogenated ether class, distinguished by an extremely low blood-gas partition coefficient that enables exceptionally rapid induction and emergence from anesthesia.
Mechanism of Action
Desflurane enhances GABA-A receptor-mediated inhibitory chloride conductance while inhibiting NMDA and glycine receptors, producing reversible central nervous system depression with rapid pharmacokinetic profile.
Application
Used in the induction and maintenance of general anesthesia. Desflurane is indicated for outpatient and inpatient anesthesia, particularly suitable for procedures requiring rapid emergence and recovery.

Desflurane-based anesthesia provided neurologic protection in a piglet model of deep hypothermic circulatory arrest. Arterial desflurane concentrations reached nearly 9 percent within 10 minutes during cardiopulmonary bypass cooling and decreased rapidly to approximately 2 percent within 10 minutes after drug withdrawal during rewarming. The desflurane group demonstrated significantly better neurologic performance scores on postoperative day 2 compared to the fentanyl group. Histologic examination revealed less tissue damage and fewer dead neurons in the neocortex of desflurane-treated animals. These findings demonstrated that desflurane conferred neurologic protection against ischemic injury associated with CPB and DHCA, suggesting potential application in infant cardiac surgery to reduce neurologic complications.
