General Description

Ritodrine hydrochloride is a synthetic sympathomimetic amine with a phenethylamine structure, containing a hydroxyl group and a methyl substituent on the ethylamine side chain. Its chemical structure is 4-[2-[[(1S,2R)-1-hydroxy-1-(4-hydroxyphenyl)propan-2-yl]amino]ethyl]phenol hydrochloride. Ritodrine is a white to off-white crystalline powder, soluble in water. It is a beta-2 adrenergic receptor agonist.

Mechanism of Action

Ritodrine acts as a selective beta-2 adrenergic receptor agonist, stimulating the production of cyclic AMP (cAMP) in uterine smooth muscle cells. This leads to a decrease in intracellular calcium, resulting in the relaxation of uterine muscle and the inhibition of premature labor contractions. The drug also has bronchodilatory effects, though these are not its primary indication.

Application

Ritodrine is indicated for the management of premature labor, specifically to inhibit uterine contractions and delay delivery in patients with preterm labor. It is effective in prolonging pregnancy and allowing for the administration of corticosteroids for fetal lung maturity.

In a retrospective cohort of 226 twin pregnancies, total ritodrine hydrochloride dose for threatened preterm labor was significantly associated with pulmonary edema (adjusted OR 1.02 per unit). The optimal cut‑off to predict pulmonary edema was 26 units (1872 mg), with sensitivity 61.3% and specificity 87.8%. Monitoring cumulative dose may help guide management and reduce this serious side effect.
