General Description

Pentolinium tartrate is a synthetic bisquaternary ammonium compound with a pentamethylene chain, acting as a ganglionic blocker. Its chemical structure is (2R,3R)-2,3-dihydroxybutanedioic acid; N,N-dimethyl-N-[5-(dimethylamino)pentyl]piperidin-1-aminium.

Mechanism of Action

Pentolinium acts as a competitive antagonist at nicotinic acetylcholine receptors in the autonomic ganglia, blocking sympathetic and parasympathetic neurotransmission. This leads to vasodilation, decreased peripheral vascular resistance, and reduced blood pressure. The drug also reduces gastrointestinal motility and secretions.

Application

Pentolinium is indicated for the treatment of severe hypertension, particularly in patients with malignant hypertension or hypertensive emergencies. It is also used for the management of hypertensive crises.
