General Description
Milrinone lactate is the lactate salt of milrinone, a synthetic bipyridine derivative that functions as a positive inotrope and vasodilator. Its chemical structure is 1,6-dihydro-2-methyl-6-oxo-[3,4'-bipyridine]-5-carbonitrile lactate. It is a white to off-white crystalline powder that is soluble in water.
Mechanism of Action
Milrinone acts as a selective inhibitor of phosphodiesterase type 3 (PDE3), an enzyme responsible for the breakdown of cyclic AMP (cAMP) in cardiac and vascular smooth muscle. This inhibition increases cAMP levels, leading to enhanced myocardial contractility (positive inotropy) and relaxation of vascular smooth muscle (vasodilation), which reduces afterload and improves cardiac output.
Application
Milrinone is indicated for the short-term management of congestive heart failure in patients who have not responded to standard therapy. It is used to improve hemodynamic parameters in patients with low cardiac output, often in the intensive care unit setting.
The MIDAS trial protocol describes a multicenter, double‑masked, placebo‑controlled RCT (n=316) testing prophylactic milrinone versus placebo to prevent post‑ligation cardiac syndrome (PLCS) or death within 7 days of patent ductus arteriosus closure in extremely premature infants. The primary outcome is a composite of PLCS or death. This will provide evidence on a treatment for a common, morbid complication.

Does Milrinone Lactate require protection from light and moisture during storage?
Yes, it is sensitive to both light and moisture. Light causes photodegradation and moisture promotes hydrolysis of the lactate ester. Store in light-resistant, tightly sealed containers with desiccant.
What is the recommended storage temperature for Milrinone Lactate?
Store at controlled room temperature (15–25°C). Avoid excessive heat above 30°C, which accelerates degradation.
Is Milrinone Lactate stable in injectable solution formulations?
Aqueous solutions are stable when protected from light and stored at room temperature.
How is the impurity milrinone (free base, a hydrolysis product) monitored?
This primary degradation product is specifically quantified using a stability-indicating HPLC method, ensuring it remains within pharmacopoeial limits.
