General Description
Desmopressin acetate is a synthetic analogue of the natural antidiuretic hormone arginine vasopressin (ADH). Chemically, it is known as 1-(3-mercaptopropionic acid)-8-D-arginine vasopressin monoacetate (salt) . It was developed through structural modifications of the native hormone to enhance specific therapeutic actions while minimizing undesirable effects.
Mechanism of Action
Desmopressin exerts its pharmacological effects primarily through its potent and selective agonist activity at vasopressin V2 receptors. This selectivity is the cornerstone of its dual therapeutic profile.
Application
Desmopressin is the mainstay of replacement therapy for central (cranial) diabetes insipidus. It is used to manage permanent disease and the temporary polyuria and polydipsia following head trauma or pituitary surgery. It is also indicated for patients with hemophilia A who have factor VIII coagulant activity levels greater than 5%.
Desborough MJ, et al. assessed the efficacy of perioperative desmopressin in reducing bleeding and blood transfusion requirements among patients with platelet dysfunction, including those receiving antiplatelet therapy. Pooling data from 10 randomized controlled trials involving 596 cardiac surgery patients, the analysis demonstrated that desmopressin significantly reduced the number of red blood cell units transfused, decreased total blood loss, and lowered the risk of reoperation due to bleeding.
