General Description

Vibegron is a pyrrolopyrimidine derivative formed through the formal condensation of the carboxy group of (6S)-4-oxo-4,6,7,8-tetrahydropyrrolo[1,2-a]pyrimidine-6-carboxylic acid and the amino group of (R)-(2R,5S)-5-(4-aminobenzyl)pyrrolidin-2-ylmethanol. Functioning as a highly potent beta3-adrenergic receptor agonist, it is characterized by its superior oral bioavailability.

Mechanism of Action

Vibegron is a potent and selective β3-adrenergic receptor agonist that targets bladder detrusor muscle relaxation, resulting in enhanced bladder capacity.

Application

Vibegron is a medication that was approved in Japan for use in September 2018. It is used to treat the symptoms of overactive bladder (OAB) which include urgency and frequency of urination. In reducing symptoms of urge urinary incontinence, vibegron can improve a person's decreased quality of life. It was approved in the United States for use in adults for the treatment of OAB in December 2020.

Vibegron is thought to increase bladder compliance through its activity as a selective β3AR agonist, leading to an upregulation of the production of cyclic AMP, resulting in the activation of the downstream signalling cascade. This appears to induce relaxation of the detrusor smooth muscle during the bladder filling phase and is associated with a reduction in symptoms of urgency, frequency, and UUI. Importantly, this activity does not appear to have any effect on detrusor motor function and does not seem to reduce voiding efficiency. This is in contrast to antimuscarinic agents which cause bladder relaxation through the inhibition of cholinergic activity.
