General Description
Protirelin is a synthetic tripeptide, specifically thyrotropin-releasing hormone (TRH) with the sequence L-pyroglutamyl-L-histidyl-L-prolinamide. It is a white to off-white powder, soluble in water. Protirelin is a hypothalamic hormone that acts on the anterior pituitary gland to stimulate the release of thyroid-stimulating hormone (TSH) and prolactin.
Mechanism of Action
Protirelin binds to specific TRH receptors on the thyrotroph cells of the anterior pituitary, activating a G-protein-coupled signaling cascade that leads to the release of thyroid-stimulating hormone (TSH). This, in turn, stimulates the thyroid gland to produce and release thyroxine (T4) and triiodothyronine (T3). The drug also stimulates the release of prolactin from the pituitary.
Application
Protirelin is indicated as a diagnostic agent for the evaluation of thyroid function, particularly for the differentiation between primary, secondary, and tertiary hypothyroidism. It is used in the TRH stimulation test, where the TSH response to protirelin is measured to assess the integrity of the hypothalamic-pituitary-thyroid axis.
