General Description
Procarbazine Hydrochloride is an antineoplastic alkylating agent and methylhydrazine derivative used in the combination chemotherapy of Hodgkin's lymphoma and certain brain tumors. It is administered as the hydrochloride salt and acts through multiple mechanisms including DNA alkylation, inhibition of DNA, RNA, and protein synthesis, and induction of chromosomal damage.
Mechanism of Action
Procarbazine Hydrochloride undergoes oxidative activation to reactive intermediates that function as alkylating agents, forming covalent adducts with DNA (particularly at the O-6 position of guanine). This leads to DNA cross-linking, strand breakage, and inhibition of DNA synthesis. Procarbazine also inhibits DNA polymerase, RNA polymerase, and protein synthesis, and may act as a monoamine oxidase (MAO) inhibitor, requiring dietary restrictions.
Application
Procarbazine Hydrochloride is indicated as a component of MOPP (Mustine, Oncovin, Procarbazine, Prednisone) and other combination chemotherapy regimens for the treatment of Hodgkin's lymphoma (stages III and IV) and is used off-label in the treatment of primary brain tumors including glioblastoma multiforme.
Procarbazine is an oral alkylating agent used in Hodgkin lymphoma and brain tumors. It is a prodrug metabolized to active intermediates that inhibit DNA, RNA, and protein synthesis, and crosses the blood-brain barrier. Early MOPP regimen achieved high remission rates but caused infertility and secondary leukemias. Recent dose-intensified BEACOPP (bleomycin, etoposide, doxorubicin, cyclophosphamide, vincristine, procarbazine, prednisone) in advanced Hodgkin lymphoma showed 5-year freedom from treatment failure of 87% versus 69% with COPP-ABVD, and overall survival of 91% versus 83%. For anaplastic oligodendroglioma, PCV (procarbazine, lomustine, vincristine) added to radiotherapy prolonged progression-free survival from 13 to 23 months. Despite past safety concerns, procarbazine remains important in modern combination regimens for lymphomas and gliomas, with optimized dosing to reduce late toxicity.
