General Description
Nilutamide is a nonsteroidal antiandrogen with a hydantoin core, a trifluoromethyl group, and a nitro group, structurally related to flutamide. Its chemical structure is 5,5-dimethyl-3-[4-nitro-3-(trifluoromethyl)phenyl]imidazolidine-2,4-dione.
Mechanism of Action
Nilutamide acts as a competitive antagonist at the androgen receptor, blocking the binding of testosterone and dihydrotestosterone to the receptor. This prevents androgen-induced gene expression, inhibiting the growth of prostate cancer cells. Unlike GnRH agonists, nilutamide does not cause an initial flare reaction, making it suitable for use in combination with surgical or medical castration.
Application
Nilutamide is indicated for the treatment of metastatic prostate cancer, in combination with surgical castration or GnRH agonists. It is used to block the effects of adrenal androgens, contributing to the suppression of tumor growth.
