General Description

Levobunolol hydrochloride is a noncardioselective beta-adrenoceptor blocking agent, demonstrating equipotent activity at both beta-1 and beta-2 adrenergic receptors. Its levorotatory isomer is responsible for the pharmacologic activity.

Mechanism of Action

The primary mechanism for reducing intraocular pressure is a decrease in aqueous humor production within the ciliary body. Beta-adrenergic blockade in the ciliary epithelium reduces cyclic AMP formation, which diminishes the secretion of aqueous fluid into the anterior chamber. In bronchial and vascular smooth muscle, levobunolol competes with catecholamines for beta-2 receptors, inhibiting sympathetic stimulation.

Application

This agent is indicated for lowering elevated intraocular pressure in patients with chronic open-angle glaucoma and ocular hypertension. It is effective in reducing pressure both in glaucomatous eyes and in eyes with normal pressure. Its nonselective beta-blocking activity contributes to its ocular hypotensive efficacy.
