General Description
Iothalamate meglumine is an ionic, high‑osmolality, tri‑iodinated radiographic contrast medium formed by combining iothalamic acid with meglumine (N‑methylglucamine). Each molecule contains three iodine atoms for X‑ray attenuation and carries a net negative charge. The meglumine counterion improves water solubility and reduces the viscosity of the concentrated solution compared to the sodium salt.
Mechanism of Action
The three iodine atoms per molecule strongly absorb X‑rays, creating contrast between blood vessels, organs, and surrounding tissues. Following intravascular injection, iothalamate distributes in the extracellular fluid and is excreted unchanged by glomerular filtration without tubular reabsorption or metabolism. Its high osmolality (approximately 1500 mOsm/kg) induces osmotic diuresis and endothelium‑derived vasodilation, which are direct physicochemical effects.
Application
Iothalamate meglumine was historically used for intravenous urography, cerebral angiography, peripheral arteriography, and contrast‑enhanced computed tomography.
