General Description

Iodohippurate Sodium I-123 (123I-OIH) is a radiopharmaceutical containing the radioisotope iodine-123 (123I) bound to ortho-iodohippuric acid, a derivative of hippuric acid. Iodine-123 is a short-lived gamma-emitting radionuclide (half-life 13.2 hours) ideal for single-photon emission computed tomography (SPECT) imaging. Iodohippurate is efficiently cleared by the kidneys through both glomerular filtration and tubular secretion.

Mechanism of Action

Iodohippurate Sodium I-123 is cleared from the blood by the kidneys via a combination of glomerular filtration (approximately 20%) and active tubular secretion (approximately 80%), primarily through the organic anion transporter (OAT1 and OAT3) systems in the proximal tubule. Following intravenous injection, it rapidly distributes in plasma, and its renal clearance pattern provides functional information about both glomerular filtration rate (GFR) and effective renal plasma flow (ERPF) when imaged dynamically.
