General Description
Fomepizole is a small molecule with a pyrazole ring and a methyl group, chemically known as 4-methylpyrazole. Its structure is a five-membered ring with two nitrogen atoms and a methyl substituent. Fomepizole is a clear, colorless liquid that is freely soluble in water. It is a potent inhibitor of the enzyme alcohol dehydrogenase (ADH).
Mechanism of Action
Fomepizole acts as a competitive inhibitor of alcohol dehydrogenase, the enzyme responsible for the first step in the metabolism of methanol and ethylene glycol. By blocking the conversion of methanol to formaldehyde and formic acid, and ethylene glycol to glycolic acid and oxalic acid, it prevents the accumulation of toxic metabolites. The drug has a higher affinity for ADH than ethanol, making it a more effective inhibitor.
Application
Fomepizole is indicated as an antidote for the treatment of methanol and ethylene glycol poisoning. It is used in patients with confirmed or suspected toxic alcohol ingestion to prevent the formation of toxic metabolites and to reduce the risk of metabolic acidosis, renal failure, and blindness. The drug is also used for the treatment of adverse reactions to disulfiram.
