General Description
Ethacrynate Sodium is the sodium salt of ethacrynic acid, a potent loop diuretic belonging to the phenoxyacetic acid derivative class. Unlike furosemide and bumetanide, it does not contain a sulfonamide moiety, making it structurally unique among loop diuretics.
Mechanism of Action
Ethacrynate Sodium inhibits the Na+/K+/2Cl- cotransporter (NKCC2) in the thick ascending limb of the loop of Henle by binding to the chloride binding site, reducing sodium, potassium, and chloride reabsorption. The absence of a sulfonamide group distinguishes its chemical interaction profile from furosemide.
Application
Indicated for the treatment of edema associated with heart failure, hepatic cirrhosis, and renal disease. Ethacrynate Sodium is a potent loop diuretic that does not contain a sulfonamide moiety, making it a critical alternative for patients with sulfonamide allergies. It acts on the thick ascending limb of the loop of Henle.
Ethacrynate sodium, administered as ethacrynic acid (EA), targets glutathione S-transferase M1 (GSTM1) to promote browning of white adipocytes. In high-fat diet-induced obese mice, intraperitoneal injection of ethacrynic acid significantly reduced body weight gain, decreased white adipose tissue volume, and improved glucose tolerance and insulin sensitivity without affecting food intake.
Mechanistically, ethacrynic acid directly binds to GSTM1, disrupting the interaction between GSTM1 and apoptosis signal-regulating kinase 1 (ASK1). This disruption activates ASK1 and its downstream p38 MAPK signaling pathway, leading to increased expression of uncoupling protein 1 (UCP1) in white adipose tissue. The diuretic effect of ethacrynic acid was ruled out as the cause of weight loss, as hydrochlorothiazide treatment did not produce similar anti-obesity effects. This study revealed a novel pharmacological mechanism of ethacrynate sodium beyond its traditional diuretic action, positioning it as a potential therapeutic agent for obesity and metabolic disorders through promoting white fat browning.
