General Description
Azathioprine Sodium is an immunosuppressive antimetabolite that functions as a prodrug of 6-mercaptopurine (6-MP). Administered in its sodium salt form to improve solubility, it is widely used in both organ transplantation and the treatment of autoimmune diseases. As an imidazoline derivative of 6-mercaptopurine, it interferes with purine nucleotide synthesis and lymphocyte proliferation.
Mechanism of Action
Azathioprine Sodium is converted to 6-mercaptopurine (6-MP) non-enzymatically in tissues, and 6-MP subsequently undergoes metabolic activation through hypoxanthine-guanine phosphoribosyltransferase (HGPRT) to active thiopurine nucleotides (TGNs). These nucleotides are incorporated into DNA and RNA during cell replication, causing DNA damage in rapidly dividing lymphocytes and inhibiting lymphocyte proliferation and function.
Application
Azathioprine Sodium is used as an immunosuppressive agent in organ transplantation (renal, hepatic, cardiac) for the prevention of allograft rejection, and in the management of autoimmune conditions including systemic lupus erythematosus (SLE), rheumatoid arthritis, Crohn's disease, and ulcerative colitis.

Azathioprine (AZA), an immunomodulator used in autoimmune disorders and transplantation, has active metabolites 6-mercaptopurine and 6-thioguanine. Recent applications include treating systemic lupus erythematosus, pemphigus vulgaris, and psoriasis. Pharmacogenomic testing of TPMT and NUDT15 genotypes minimizes myelosuppression risk.
