General Description
Prednicarbate is a synthetic corticosteroid of the diester class, belonging to the glucocorticoid family of steroidal anti-inflammatory agents. Its structure features a prednisolone skeleton with a 17-propionate and 21-ethyl carbonate esterification, which modulates its topical potency and pharmacokinetic profile. The diesterification contributes to enhanced lipophilicity and targeted local activity with reduced systemic absorption.
Mechanism of Action
Prednicarbate acts through binding to the glucocorticoid receptor (GR), translocating to the nucleus, and modulating the expression of anti-inflammatory and immunosuppressive genes. It inhibits phospholipase A2 (via lipocortin-1 induction), reduces the synthesis of prostaglandins, leukotrienes, and other inflammatory mediators. Its relatively selective local activity at recommended concentrations minimizes systemic glucocorticoid side effects.
Application
Prednicarbate is indicated for the topical treatment of inflammatory and pruritic dermatoses responsive to corticosteroid therapy, including atopic dermatitis, eczematous dermatitis, psoriasis, and allergic contact dermatitis.

Prednicarbate, a poorly water-soluble corticosteroid for atopic dermatitis, was loaded into mesoporous SBA-15 silica using ethanol as a safe solvent. Characterization by nitrogen adsorption (BET/BJH) showed a 24% decrease in surface area and 16% decrease in pore volume, confirming drug encapsulation inside pores without altering pore size. FTIR, elemental analysis, and thermogravimetry indicated ~5% drug loading.
XRD and DSC demonstrated that encapsulated prednicarbate remained amorphous, with no crystalline drug phase. Thermal analysis also revealed a protective effect, reducing the drug’s thermal decomposition rate. A validated HPLC-MS/MS method quantified drug release in Franz diffusion cells. The SBA-15 formulation released prednicarbate six times slower than a conventional cream, providing a modified release profile. This system offers potential benefits: improved chemical stability, reduced metabolism by cutaneous esterases, lower dosing frequency, and decreased risk of corticosteroid-related adverse events such as skin atrophy.
